The National Health Mission (NHM), Uttar Pradesh invites online applications for 06 Month Certificate in Community Health for Nurses (CCHN) Training for the 2021-22 session. This course is a part of initiative under the Ayushman Bharat Scheme, GoUP envisioned to strengthen Sub-Health Centers as Health and Wellness Centers (HWCs) for improved implementation of public health programs and to enable comprehensive primary healthcare service delivery, including disease prevention and health promotion. Candidates who successfully complete this course and pass the final examination will be posted as Community Health Officer (CHO) at Sub-Health Centers level HWCs on contract basis.

UP CHO Educational Qualification:-

General Nursing and Midwifery (GNM) from a recognized Institute or B.Sc. Nursing or Post Basic B.Sc. Nursing from a recognized Institute/University.

UP CHO Age Limit:-

Maximum Age: 35 years
35 years of age as on date i.e. 20th July 2021

Computer Based Test Exam pattern

Section-I	Professional Knowledge (Discipline related)	80 Marks
Section-II	General Aptitude, Reasoning, General Awareness and Basic Computer knowledge	20 Marks

100 multiple-choice questions (MCQ).
Each question will carry 1 mark and there is NO NEGATIVE marking for wrong answer.

Minimum Qualifying marks in the Computer Based Test:-

GENERAL(UR) / EWS	Minimum 33% marks i.e. 33 Marks out of 100
OBC (Non-Creamy Layer)	Minimum 30% marks i.e. 30 Marks out of 100
SC / ST / PwD	Minimum 24% marks i.e. 24 Marks out of 100
